I love JotForm, it's so easy to use. I did several one page forms, over the last few years; last year I used it to manage registrations for my class reunion - thanks to the payment option; and this year I'm using it to create an application form for our students. Thanks to the conditional logic, they will have the ability to upload files. Did I say I love this software?
