Configuring a PACS environment from scratch for a mid-sized radiology department is rarely fun, but PaxeraHealth made it less painful than I expected. The permissions model is genuinely well thought out. Granular role assignments, department-level access controls, and the ability to restrict specific modalities to specific user groups all worked the way I hoped without me having to dig through documentation for hours. My team of three got the core admin structure standing in under two weeks, which felt like a win.
My one real frustration is the configuration interface for user group inheritance. When you're nesting groups, the UI doesn't always make it obvious which permissions a child group is pulling from the parent, and I had to run a few test accounts to verify things were set up correctly. Their support team answered my ticket promptly, so it wasn't a crisis, just friction I didn't want. If you're evaluating this for a department with complex permission tiers, budget a little extra setup time there.
